Any time I start drinking it doesn't take long before that dull ache appears. It scares me every time it comes back and every time I go running to the doctor. Tests always show up fine, including ultrasound and blood work. That doesn't mean something else isn't happening.

The liver itself doesn't do a great job of conveying pain.
It's the symptoms associated with liver damage / failure that get people. Things like portal hypertension, ascites, jaundice, esophageal varices, and other organ failure due to toxins remaining in your bloodstream.
Any amount of alcohol will irritate liver cells. But alcoholics tend to go way overboard and going way overboard repeatedly can cause permanent liver damage. Be nice to your liver - it lives up to its name. A very useful organ indeed.
