So basically this medicine is only good if the alcoholic is willing to take it and takes it as perscribed? Is this the medicine that makes you very sick if you take it and drink alcohol.

Yes, and just like the alcoholic who drinks even though the drinking makes him sick, the drinker on antabuse can drink when hes on that also...thereby making him sick.
None of that means anything unless the alcoholic wants to stop drinking.
