Good call Stewy, glad you going to seek some help from your doc.

As far as how you start the conversation, just tell them the truth. You have a serious drinking/addiction problem and also anxiety issues so just tell them that. They will know what questions to ask from there. They are there to help you, so you have to tell them what you need help with and there is no sense beating around the bush. If you had a broken arm you'd say that you think you have a broken arm, right?

Don't worry about feeling ashamed or embarrassed, they have heard this story many, many times. I told it to my doctor the same thing and was treated with respect and compassion.
