It may be possible that the feeling of unreality is related to the naltrexone. You might want to search on this site or elsewhere to see if others have experienced that when they started on the medication. Better yet, give your psychiatrist a call.
I wanted to go the naltrexone route when I was still drinking (read intriguing things about The Sinclair Method) but wound up in a program that uses antabuse. That works for me. And I think I'm better off this way, at least for now.
