Well I'm not talking about someone with a healthy appetite. I'm talking about engaging in extremely high risk behaviors which create a very real adrenaline rush. It is often not about what "turns someone on" in the tradition sense, but rather a compulsion that ends up creating the same pain as the cycle of drinking or using.
