Have you sat down with her while she is sober and had a seriouos discussion about your worries and fears? Maybe you should do that and this time tell her what you are prepared to do to make some changes. Of course, you'll have to figure that out first, but instead of just going on the same way after a discussion, maybe having some clear cut boundaries in place and telling her how things are going to change might let her know that things will not continue the way they have been going, and convey to her the seriousness of the situation.
