My worse feeling is if he eventually does drive and severally injures someone or worse.

You have confronted him, communicated your concerns, asked him to address it and get help, he's not responding. YOU are at risk if you continue in business relationship with this person. His actions threaten your interests and your livelihood. Depending on the measures you've taken to protect yourself - you could wind up personally liable for his actions if he, say, kills someone driving a company-owned vehicle and you knew of his problem.
If I were you, I'd get an attorney engaged and set about dissolution of this partnership and get on with your life before he gets on ruining it for you.
