A second opinion sounds like a good idea. I know you don't want to sit in the ER, however, if you are really struggling it might be the best place.
You mentioned you have good insurance, most insurance policies have a specific addiction department. You may want to look into that and give them a call. They specialize in helping people kick their addictions, and there won't be any judgements.
