Introduction
Hi everyone. My name is Melissa and I'm a grateful alcoholic. I'm very glad to be here.
I found the site while searching the 'net for "Step 9 Template", since my sponsor has given me a very loose one and I wondered what others have used. I read the discussion that largely says "you don't need one", yet I've found that it's helpful to read examples.

I suppose people just format their step 9 whatever way they feel works for them personally. I also 'shopped around' for step 9 ideas by asking others how they did it. My sponsor laughed at the 'system' I finally adopted for Step 9, but also saw it worked for me.
I bought a pack of box file cards (the ones that are landscape and approx 3 inches high by 5.5 inches wide). I completed one card for each amend I needed to make.
On the top line (in green) I wrote the name of who the person was eg. Dentist
A couple of lines down (in orange) I wrote the nature of my harm eg. Financial- I never paid my bill
A couple of lines down (in bright pink) I wrote down what action I needed to take eg. Go and pay it
I liked the fact that I could easily sort them (either into now; later; never piles, or just done and to-do piles). I didn't ever buy a box file for them. I just keep them in a make-up / pencil case (not sure which of those it's supposed to be, but it's a good size for them).

In the last several months I attended a back to basics weekend workshop. All 12 steps are done based on how the original meeting / newcomers were taught to do them.
Here's a link to the back to basics. This includes forms and explanations of what was done. A 4th step form is provided and then used for the 9th step amends.
AA Back to Basics - It's all about saving lives!
"AMENDS: Referring to the 4th Step, If a name has more than 1 or 2 marks under harms they are likely due amends.
Put an A in the column and ask “How can you make amends to this person?” Make sure to review the amends
being made to insure that they are appropriate. Plan to discuss the amends before and after they are made. Step
Nine is an ongoing process. Living in God’s will, cleaning up the wreckage of the past and helping others repays our
debt, “we are reborn”.
