Adult Children of Alcoholics [ACOA] Issues Checklist
The following checklist is designed to assess many of the attributes and issues frequently associated with Adult Children of Alcoholics (ACOAs), including: Low Self-Esteem, Perfectionism, Conflicted Personal Relationships, Need for Approval, Impulsivity, and Sublimation of Personal Needs. All items are phrased in the same direction and the following cutoff scores are proposed for interpretive and diagnostic purposes: 0-15 No significant concerns associated with being an ACOA
16-25 Minimal concerns associated with being an ACOA
26-34 Serious issues associated with being an ACOA
35-50 Severe issues associated with being an ACOA
Please note that many ACOAs usually manifest either a perfectionistic, rigid style or a disorganized and impulsive one. Although it is unlikely that any single client will manifest both of these styles, both styles are included here. We also recommend that users of this instrument remove or white-out the sub-section headings before administering the checklist to clients, because the heading titles may be interpreted negatively by some clients, which could affect their responses.
[A] LOW SELF-ESTEEM
A1. I often feel that I am unworthy person.
A2. I don't expect people to understand me.
A3. There are very few people in the world who really care about me.
A4. Sometimes I think I do not deserve to be loved.
A5. I don't deserve to be happy.
A6. I feel that I am a weak person.
A7. I worry that people will think I am incompetent.
A8. Success is easier for most people than it is for me.
A9. I fail at most things I try to do in my life.
A10. I have trouble believing people when they tell me they like me.
[B] PERFECTIONISM
B1. I take my responsibilities very seriously.
B2. I hate it when I don't have things under control.
B3. I frequently take on too many tasks for me to be able to reasonably complete.
B4. I don't laugh very often.
B5. I often become upset when there is an unanticipated change in plans.
B6. I am considered to be responsible to a fault by those around me.
B7. People often tell me I take things too seriously.
B8. I tend to be very perfectionistic and want everything to be just so.
B9. I don't understand how some people can relax so easily.
B10. I frequently overreact to even the smallest change in my routine.
[C] CONFLICTED PERSONAL RELATIONSHIPS
C1. My family and I have a lot of conflicts and arguments.
C2. I often get "sucked in," even when I try to avoid arguments with family members.
C3. I have trouble staying in relationships for long periods.
C4. I have trouble communicating with people I care about.
C5. I'm not sure what a "normal" relationship with a significant other would look like.
C6. Sometimes it's hard for me to "connect" with other people.
C7. I don't really know what a "normal" family looks like.
C8. I often feel that I am "different" from those around me.
C9. Sometimes I wonder what it would be like to be happy.
C10. I am very loyal to friends and family, even when they don't deserve it.
[D] NEED FOR APPROVAL
D1. I am afraid of what others think of me.
D2. People describe me as someone who can "smooth over" conflicts before they escalate.
D3. Other people's opinions of me are very important to me.
D4. I avoid conflicts at all costs.
D5. I sometimes find myself lying to other people for no real reason.
D6. I will give up on something I want to avoid having a disagreement with someone.
D7. I worry that other people don't like me.
D8. I would rather lie than tell someone the truth and make them angry.
D9. I've been known to back down from something I believe in so someone else won't get
upset with me.
D10. It is important to me that most, if not all people in my life, like me.
[E] IMPULSIVITY
E1. Folks around me say I am irresponsible and apt to get into trouble.
E2. Sometimes, I behave impulsively resulting in problems for myself and others.
E3. I am in my element when things are disorganized.
E4. I don't usually pay attention to details when working on tasks.
E5. I often do things without thinking about the consequences.
[F] SUBLIMATION OF PERSONAL NEEDS
F1. I tend to take care of others peoples' needs before my own.
F2. Members of my family often make demands on me and expect me to meet them.
F3. I feel guilty whenever I do something nice for myself.
F4. I usually ignore my own preferences in favor of those of people around me.
F5. I try to satisfy requests made by family members, even when it is difficult for me to do
so.
