• Tell him or her all the **** you've told us
• Do what he or she says

If he's a lazy worthless doctor with regard to boozing, then proceed to Plan B:

• Go to another general doctor
• Tell him or her all the **** you've told us
• Do what he or she says

If he too is a lazy worthless doctor, then repeat Plan B often as necessary.

Seriously, not trying to make light of your situation. You've got **** going on that not even a doctor could diagnose you properly over the internet, let alone a bunch of drunks - you need to see a medical professional.
