For a quick start I'd suggest you find some NA meetings in your area, even if you ultimately choose a different recovery route, the face to face support of others and a set time and place to go can be a huge help at the beginning.
Please see a dr, be totally honest and find out what they recommend. Many communities have substance abuse resources, you can find out about them at clinics or by calling a substance abuse hotline.
You don't have to do this alone. It sounds like you may have a possible support in your mom. Maybe she can watch the kids so you can make appts and meetings?
