Here's a pretty good screening tool for mental health issues.

I do suggest if you use it, you save it often, cuz it can loose data easily....
But, it's quite complete and has a print out for your health care provider. That can help start a conversation, or build upon one already begun.
It also has guidance options and their meanings.
I think it's pretty cool and a good tool also. Would love to hear your thoughts too....
