Hi Hyperman and hope you're feeling well now. At least for the moment. You need a HOSPITAL based detox/rehab. They will give you a complete work up, coordinate the various Drs., therapists etc. and give you a comprehensive report on what you need to take, what drugs are interacting with others and what you can eliminate or substitute. Sounds like your life is a living hell and it shouldn't be. You absolutely have to tell them about the street drugs you're taking. Once you get straightened out you can always score some pot or whatever. For now, and I mean today, get yourself in a HOSPITAL where a team of experts can figure this out. You'll have to be completely honest with them but the alternative looks like death if you don't do this. At 45 you have a lot of years left to suffer or feel OK. Your choice entirely.
