The biggest risk for an otherwise healthy person (or as healthy as anyone using heroin on a regular basis can be) is dehydration. Even if it's coming out both ends, gatorade or something else that replaces salts is important.
Other than that, immodium for the diarrhea, advil or tylenol for the body aches, lots of hot showers and/or baths, and a good supply of clean sheets & p.j.s is about all you can do for him or he can do for himself. I don't know how long he was clean before this relapse, but withdrawal tends to get worse each time. On average, 3-5 days gets most folks over the hump, and if he was clean awhile before the relapse and the relapse lasted only a month, that should be long enough to get him out of the woods.
As painful as detox is, the tougher part is staying clean. If he were asking me for suggestions, the best one I would give him would be to get his butt to a meeting as soon as he can keep said butt out of the bathroom for an hour.
Peace & Love,
Sugah
