The 'Insert Image' button is for posting an image via a web address.

The buttons on 'Reply to Thread' don't work, but you can set your fonts and quotes and stuff in quick reply and then go advanced if you need to add an attachment. Like the rest of the buttons on top, the paper clip doesn't work, but if you scroll down to 'Additional Options' and click 'Manage Attachments', you can upload your picture. Once you've done that, your image is attached and will show in your message or message preview.
